Bioidentical hormones in Amarillo: what it means

"Bioidentical" is a word compounding pharmacies often use in marketing, but what it describes is real: a hormone built like the body's own. Estradiol, the main estrogen the ovaries make before menopause, and micronized progesterone both fit that description. FDA-approved versions of both are sold by prescription at retail pharmacies.

"BHRT" often means compounded hormones, a cream, a blend such as Bi-Est or a pellet that a pharmacy mixes for one prescription. Compounded drugs are not FDA-approved, so the FDA cannot assure their safety, effectiveness or dose. The FDA says it has no evidence that compounded "bioidentical hormones" are safe and effective, or more effective than FDA-approved hormone therapy. When the National Academies reviewed them in 2020, the evidence was mostly low quality, such as anecdotes and patient reports.

42 OB-GYNs in the federal NPI registry give a primary practice address in Amarillo: 20.9 per 100,000 residents, above the US rate of 17.8 and above Texas's 15.4, and more per resident than 52% of the 924 cities VevaMD covers. That is about 520 women aged 45 to 64 for each OB-GYN in the city, a ratio rather than a count of open appointments.

Nearly all of the area's OB-GYNs are in Amarillo itself: 42 practice within 25 miles, 15.9 per 100,000 of the 264,208 residents of that radius. None of this replaces your own OB-GYN or a menopause doctor you trust; VevaMD adds an online route, with a physician licensed in Texas reviewing your answers from home in Amarillo.

Through VevaMD, a physician licensed in Texas decides whether hormone therapy fits you and, if so, the form: an estradiol pill, patch, cream or vaginal cream, with progesterone if you have a uterus. There are no pellets. Because the dispensing pharmacy also compounds, you can ask whether your estradiol or progesterone is an FDA-approved product. Testosterone cream for women and Bi-Est are compounded and not FDA-approved.

A pellet clinic is a different model: it implants a hormone pellet under the skin during a visit, and compounded pellets are not FDA-approved. The Endocrine Society cites blood levels well above the expected range in women using compounded pellets. ACOG's guidance is to prefer FDA-approved menopause hormone therapy to compounded hormones.

Is bioidentical hormone therapy (BHRT) available in Amarillo?

From home in Amarillo: a physician licensed in Texas reviews a private intake and decides whether estradiol, with progesterone if you have a uterus, fits. Compounded "BHRT" blends are not FDA-approved; ask whether your prescription is an FDA-approved product or compounded.

What is the difference between this and BHRT pellets?

Pellet clinics place a hormone pellet under the skin; compounded pellets are not FDA-approved. VevaMD's physicians prescribe pills, patches, creams or vaginal forms that ship to your door in Amarillo, and do not use pellets. Some of those forms are compounded too, so ask which yours is.
